Dinner Journal Ad

Please complete the ad blank form below to submit an ad and/or dinner reservation online to YBH-Hillel.

Note: Upon submission of this ad, you will be redirected to a page where you will have the option of paying for your ad or dinner reservation online.

NOTE: If you only intend to attend the dinner and will not be placing an ad, you must select "no ad" and complete the dinner reservations portion of this form.

Reservations are $250 per person. A contribution of $600 or more entitles you to an ad plus (up to) two dinner reservations. Please contact the school for reservations for five or more.

Please check the box if your reservations are to be in addition to the one or two reservations you are entitled to as part of your ad purchase of $600 or more. For example: Reservations for one ($250) plus the two reservations from an ad purchase of $600 or more will result in three reservations being made on your behalf for which you will be billed a total of $850. You do not need to check off the box if you are only making the one or two reservations you are entitled to as part of your ad purchase and no additional reservations.
